By signing digitally below you agree and expressly consent that your son or daughter can receive emergency medical treatment from any physician, hospital or other medical center without the necessity of them first notifying the emergency contact above.  You also agree to hold harmless any chaperone, minister, director, administrator, deacon or elder of the Missouri St. Church of Christ, physician, hospital or other medical center for rendering of such services.  You authorize the use of any photography and video taken of your child listed above to be used in any and all social networking sites managed by Missouri St. Church of Christ.  You are also agreeing to hold harmless any Missouri St. Church of Christ volunteers and staff should an injury or accident occur with the child listed above.  By signing below, you hereby authorize any administrator of the Missouri St. Church of Christ to act for yourself according to his best judgement in an emergency requiring medical attention.  Missouri St. Church of Christ is not responsible for lost or stolen personal items.  By signing digitally below, you are allowing the child listed above to attend Keepers of the Kingdom Vacation Bible School at Missouri St. Church of Christ in West Memphis, AR on June 11-14, 2023.  Please check the terms & conditions as well as sign digitally if you agree to these statements and terms.
